K.O.R.S. Membership form 2005

The Keystone Off-Road Society is a Non-profit organization that promotes Off-road sports. As a group, we promote several mountain bike races, youth projects, as well as maintain the NEW Keystone mountain bike and hiking trails. Opened last November. Join up and help support Mountain biking in Oklahoma. It is time for the Tulsa area off road cyclist to unite and form a group that will advance the off-road endeavors.

Waiver of claim:

In consideration of membership in the Oklahoma Earthbike Fellowship and Keystone off Road Society (hereinafter referred to OEF/KORS). I, for myself and my minor child/children, heir, executors, administrators and assigns, hereby agree to forever release and discharge any and all injuries, including death and any property damage in any manner arising or resulting from my participation or my child/children participation in any activity conducted by or in conjunction with OEF/KORS.

I attest and verify that I have full knowledge of the Ricks involved in mountain bike riding and in all OEF/KORS activities, that I assume those risks, that I will, without limitation, assume and pay any and all medical and emergency expenses incurred on or by child/children's behalf in the event of an accident, injury illness, or other incapacity while participating in any OEF/KORS activity, regardless of whether I have authorized such expenses. I further agree that in the event I require medical or surgical treatment while under the supervision of OEF/KORS or any of its representatives, such KORS representitive may authorize medical treatment for myself.

I have read and agree with all the terms of this waiver. (Applicants under the age of 18 require the signature of a parent or legal guardian)
